Palliative care

We have CRNP seeing patients families and counseling on plan of care and asnwering questions regarding patients condition/what to expect.

What E/M code would you suggest we use for this? These would be time based visits and typically patients are not present during this session. Is this billable at all?

I am going with "NO". Most insurances do not pay for non face to face services and the counseling with the family should be included with payment for services already rendered. This is the problem I'm having with our palliative Dr's that do not document a F/F on the days they do family meetings.
I'm curious for other responses on this too.
