All of my inpatient palliative care charges are being denied if another dr sees the patient on the same dos. I'm using Z51.5 on the charge as well as the symptom diagnosis codes. Does insurance require records on these? Is there a palliative care modifier I should be using? Any help is greatly appreciated.
 
Hi. I have had some denials like this too. The rep tells me it denied because another Dr billed the exact same cpt and Dx for the same day. I would think that submitting an appeal with the records for the visit should work. But I also would think that using the dx code Z51.5 would have worked too. Did you already come up with a solution to this?
