Hi I need help - pt presented for follow up to the office- physician reported:
34 year old female is here for annual women’s health exam. Denies gynecologic, urinary, or bowel complaints. Reports doing well, denies any family history of cancer. and added : Z01.419, N76.0, E55.9, E61.1 as DX
now since this was a follow up visit and is consider a preventive visit, should I use 99396 (mod25) + Q0091. Or just leave it as as a 99396 with Z01.419?
