Does anyone have coding guidelines they can reference on Pap smear coding?

My question is how to diagnosis code a patient with a previous history of abnormal pap smear but normal annual pap smears for the last several years.

Or, can a Pap smear with annual preventive exams return to screening if there have been a series of normal Pap smears? Or must there always be the history of abnormal pap smear diagnosis, even if having three years of normal paps, making the test no longer preventive?
