If a doctor sees an inpatient (who is uncircumcised) and during the exam they reduce the paraphimosis, can 54450 be charged without going to the operating room along with the consult?

54450 will required under general anesthesia so it has to be in OR setting. You can not use Consulting codes. You can use Inpatient codes with modifier 25 and most importantly, physician notes have to stated significantly separate from 54450.

If the physician just says I was able to reduce the paraphimosis at bedside during the physical examination. Would that just be bundled into the e/m code then? They actually have to take the patient into the operating room to charge the 54450?

I have never seen where it says 54450 needs to be in the OR under general anesthesia?
