Need some help.

(first return to surgery)
pre and post procedure dx: Complicated diverticulitis with prior temporary abdominal closure.

Procedure performed:

1. Limited abdominal exploration and inspection
2. inspection of the ostomy
3. partial fascial closure
4. temporary abdominal wall closure with skin flaps

Need help with CPT code or codes.

(second return to surgery)
pre and post procedural dx: open wound, prior surgery

procedure performed:

1. limited exploratory laparotomy
2. fascia closure with mesh
3. wound VAC placement

Need help with these CPT codes also.

I believe I would use a 58 modifier with the CPT code or codes but can't seem to find the correct codes to use.
