If the documentation states that patient has no history of allergies or asthma would that be considered PMH even though the pt did not have they conditions?
 
This would be a determination made by the context in which the information was noted. It could be applied to two different areas:

* Review of Systems = Allergic/Immune and Respiratory
* Past Medical (PFSH) = Prior illness and Allergies

Either area could potentially be supported. Again, I would look at the context in which the information was documented.
