I need to know how would you code these pathology reports:THis is from September 2015 need ICD 9 code

1. Body of stomach biospy showing moderate acute and chronic inflammation with the presenece of intestinal metaplasia consistent with Atrophic gastritis . Theres is no evidence of helicobacter organisms (see comment)

comment:
the above body of stomach biopsy will be submitted for immunohistochemical prognostic markers to rule out any dysplasia , and separate report to follow

second report:
the findings are consistent with intestinal metaplasia of gastric mucosa with increased proliferative activity. No other adverse prognositc factors are present . Endoscopic surveillance can continue on a routine basis . review of H & E section does not show dysplasia
 
I would code for the intestinal metaplasia of gastric mucosa.
