I work a for dermatology office and our pathology is all performed by pathologist within our office. we are receiving denials from strictly Humana for cpt codes 88342 and 88312 when billed along side code 88305. Humana is paying for the initial code of 88305 and denying all additional staining. Has anyone else been seeing this with Humana? The denial code is c0-45
