Wiki Patient payment with ABN

When a Medicare patient signs an ABN can we collect payment from the patient before Medicare denies the claim? Or do we have to wait for the denial and bill the patient?
Also do we send the ABN along with the claim or do we just keep it until Medicare asks for a copy?

You would not bill Medicare at all if the patient signs the ABN. Now if patient demands a bill be sent in you can use an appropriate modifier if patient demands the claim to be submitted anyways or if there is a secondary involved.
