Hi - I received a message from a patient concerned that someone else was able to view his PHI in the patient portal. It turned out that the email address linked to his patient portal account was that of another patient. We did not see activity in his portal (reviewing for MU standards) but with the other patient's email address linked, it was possible that the other patient was notified of additions to the first patient's information. Has anyone else ever encountered a situation like this and what actions were taken in addition to correcting the email address?
