Patient Signature Financial Agreement

Does anyone have National/Ohio State guidelines for patient signatures and how often they must sign the Financial Agreement? I have the HIPAA documenation that states Notice of Privacy must be done at the first encounter with a patient and given to the patient as a reminder every 3 years. I think the Notice of Privacy includes the Financial agreement. Can someone please clarify if the documentations are one in the same.
