Provider did a full history, ROS, and Exam.

Would I bill 99385 for this?
 
what if he wanted a "annual exam/physical" before going on vacation. Since they're entitled to this once a year anyway.
 
only if the doc met the requirements for a preventive. And if this is a medicare patient, medicare does not pay for preventives only AWV's which has its own guidelines as far as doc.
