Wiki PE with E/M injection and smoking cessation counsel

I am trying to bill for a PE with an office visit, TDaP injection, admin of injection and smoking cessation. Would the correct billing be as follows:

99395-51
99215-25
90715
90471-59
99406 (originally billed with modifier 59, removed and it paid)

Any help would be greatly appreciated, as I have tried multiple ways of billing this claim only to have it denied as bundled for the PE

I would recommend the following:

- 99215-25
- 99406 or 99407
- 90715
- 90471

a modifier 59 is not needed or appropriate on 99401. Unless you are completing a full preventative exam, the use of 99395 would be incorrect. 99406 and 99407 are the appropriate codes for smoking cessation.
