How do you bill insurances for the administration on the below codes?

90670
90681
90723

hi Sara, heres how i would do it

1. 90645-1 component
2. 90670-1 component
3. 90681-1 component
4. 90723-5 components

90460-1,2,3,4 QTY-4
90461-4 QTY 4

im not too familiar with the 90645 or the 90681 but it looks like they are one component each in the cpt book.
