We are a pediatric practice and one of our patients has Medicare as primary to their Medicaid. I have never billed to Medicare for an sick office visit, where do I send a paper claim? I can't send it electronic as we don't have Medicare set up in out PM system. Can anyone give me some guidance. I checked CMS website and couldn't locate the info I need where to send the claim.
