Hello,

I would like some advice on how to code the following procedure. The physician submitted a 54162 for lysis/excision of post circumcision adhesions.

"Informed consent was obtained he was brought to the operating room anesthesia was induced a timeout procedure was performed correct correct patient procedure we prepped and draped the penis in sterile standard fashion we then performed a penile block we identified multiple suture tracts around the circumference of the prior circumcision we came underneath each of the suture tracts with a straight Jake and then sharply divided the overlying epithelium. In total there were 12 suture tracts which were 1mm in length we then cauterized the base and ensured hemostasis we copiously inspected the penis to ensure that we had unroofed of suture tracts. Dermabond was applied to the incisions and the procedure was concluded he was brought to recovery room in stable condition".

I was leaning towards 10120 as upon researching I found that suture tracts are where sutures are placed and do not dissolve and they have to be removed at a later date by incision or unroofing. The physician when queried stated "we really unroofed the tissue as opposed to excising the tracts", so now I'm confused again as to whether I need to look at a different code option since the sutures were not removed. However he does state in the op note that he came underneath each of the suture tracts and divided the overlying epithelium.

Thank you
