peg changes in office

We are going to start doing peg changes in our office I am curious how to bill for that do you do an E/M code and then a peg change code and use a modifier on the e/m code?
 
If the patient is coming in just for the peg then you wouldn't code an em with the procedure. You have have medical necessity for coding the ov with a modifier. Look at 43760. I think that's the code. Sorry don't have a book with me.
