Targeted sonographic evaluation of
the chest demonstrate known collection. An 17-gauge coaxial
introducer needle was advanced under ultrasound guidance. Through
this an 18-gauge Bard biopsy device was advanced under image
guidance and biopsy was attempted. After 2 passes tissue was
noted to be very friable. Limited specimen was sent for
pathology. Decision was made to attempt drain placement. Through
the coaxial introducer needle a 035 J-tipped wire was advanced
and coiled in the cavity under ultrasound guidance. An10 French
Dawson-Mueller drain was advanced over the wire and coiled in the
cavity. Aspiration noted to be chronic appearing blood products.
Drain was secured to the chest with Prolene stitch and attached
to a JP bulb. A sterile occlusive dressing was applied.
