Would anyone be able to help me find codes for this? Provider documented:

"Has a hx of CVA 3 mo into gestation that caused cerebral palsy, left infarcts in his brain, and effected his eyes and parietal lobes and was not dx until 6 weeks old. He denies wearing glasses or having other problems reading. Mother reports visual field defect in LLQ of both eyes"
