I code for family practice clinic. We have a patient that presented with pain, swelling and redness above the knee. It was diagnosed as bursitis, however the patient has had DVT in the past and takes Coumadin daily to prevent a reoccurrence. I think it is import to include this diagnosis as it could affect the patients treatment. I am just not sure weather this is considered an active problem b/c he still receives medication for this or whether it would be considered history of b/c he no longer has a DVT, but medication is to prevent reoccurrence?

Please help...

The personal hx code works well here as the px does not have DVT. But it is still common for them to be ongoing preventative treatment. Z86.718
But if the condition is chronic I would suggest the I87.50 code instead
