Hi Everyone! My name is Carrie. I am currently enrolled in the CPB course. My pharmacy has branched out into DME. That is going pretty well so far. I have learned a lot :LOL:

My employer is getting ready to add another revenue stream with Clia Waived testing. He has his fed number and is in process with state regs. In order to be proactive and hopefully hit the ground running when it's a go, I hope y'all can see if I am on the right track.


I understand on CMS1500, I need to include our Clia # in box 19.

If we want to bill for a test for Influenza A & B, I would use dx code J11.1, CPT Codes 87804-QW and 87804-QW-59. I find our rate is $16.55 x 2 on the 22 Clinical Diagnostics Lab Fee Schedule.

Question 1: Is this correct? Missing anything?

Question 2: If the test is actually negative, do I still use that dx?
