Hi everyone, I am still stuck on phone call billing..any help with these examples much appreciated

1) phone call to pt on 3/26: pt status post Urolift Jan 2020, still experiences frequency and urgency..has had several office visits showing low post void residuals.
Plan: trial levsin nightly, sent to pharmacy, pt is to call next week with reponse

2)phone call with pt on 4/6..the trial of levsin was not met with significiant success..he has clearly failed Urolift, offered tamusulosin trial or green light, patient elects tamsulosin
patient is to call or followup with an appt after trialing the new med

Do either of these qualify as a phone visit ? G0212 ?

With the information provided, the documentation does not support billing either telephone visit 99441-99443 or G2012. The time spent is not documented, which is a basic requirement for these codes.
Even if the time was documented, I would question whether this was included in global postop since it notes a procedure was done Jan 2020.
