Does anyone have an appeals template to use for when a physician performs a physical and a separately identifiable visit that's unrelated?

AAFP has a great, very detailed letter. Check out this article: http://www.aafp.org/online/en/home/publications/news/news-now/archive/cptcodes.html - at the bottom of this article is a link to the letter.
It's very long and might not be exactly what you are looking for, but you can customize it to make it work for you!
Who is it that's denying? I'm just curious.

Thank you for your reply! This letter will definitely help out a lot. For now the only one I've seen is Aetna. Let's hope the rest pay with no problems.
