I have an internal med doctor that wants to bill out an established code (99211-99215) for a Medicare patient who needs to have a physical exam prior to entering a nursing home for Respite Care for 12 days while the patient's son goes on vacation. He wants to use dx 401.9, hypertension, in reading the office notes the condition is stable, she has no chief complaint and a form is completed for the nursing home, I am struggling with billing an E&M as it seems like an annual visit but the patient is not due for an annual. My thought is this exam, should be self pay, any ideas?
 
Sounds like a screening exam to me. Is this a medically necessary visit or one performed simply to satisfy some regulatory requirement? Sounds like the latter to me.
