Wiki Physical therapy insurances that only pay for evaluation on first visit

I'm having a hard time trying to find information about which insurances will only pay for the evaluation code for the patient's first visit with physical therapy. Does anyone know what the insurances are or where I can find that information? Thank you!
 
I have found that you need to look into each of the insurance companies you accept at the practice because they all have different rules/guidelines. There have been some that will accept an Initial Evaluation with Therapy in the first visit but I have mostly seen that they don't. I work with a Pediatric Therapy clinic in Florida so I know they will not have the same rules/guidelines as adults. I don't think there is a list anywhere of what insurances will allow or not allow billing therapy along with that first visit.
