Brand new to P/T world. I've done my research but am still questioning the proper documentation required. Which is the proper way to have daily therapy documented. If the provider does not have "total treatment time" as shown below, is it acceptable to code and bill:

97110: therapeutic exercises right upper extremity 15 mins
97110: therapeutic exercises left upper extremity 15 mins
97014: electrical stimulation (unattended) 8 mins

or does the provider have to do the following:

97110: therapeutic exercises right upper extremity 15 mins
97110: therapeutic exercises left upper extremity 15 mins
97014: electrical stimulation (unattended) 8 mins
Total treatment time: 38 mins
