PT, OT and ST Coding questions

Hi Shellie,
I just started billing for an outpatient rehab for PT,OT,and ST. I have never billed for these services before and find it a little confusing. They are not doing well on reimbursement so I hope Im able to help. As of now the thing that Im dealing with is how to bill a procedure as timed visits instead of the 15 minute units. Is there a modifier to tell the insurance the difference between the two? For commercial insurance Im billing the units and seems to be ok. However for Medicaid ( Amerigroup) the auth is for 30 to 45 min. per visit. When billed as 3 units we get denied for too many units. Any help is appreciated.
