I need some guidance please. Does the 3 of 3 PFSH only apply to Medicare? Can someone direct me where I can find this information? Thanks! :confused:
 
CPT definition

Please look at the 2009 CPT Professional Edition page 7 (green sheets), top of left column titled "Determine the Extent of History Obtained."

Under Comprehensive: ... complete past, family, and social history."

This is a CPT definition of a complete history. Applies to everyone.
