If a physician's office sends labs out to an outside lab but the physician's office is billing for the service, would you still use place of service 11? Would box 20 "outside lab" be checked yes? Any modifier? and box 32 would be laboratory information... Is this correct?

You would still use office as the location, but be sure to use modifier -90 on the labs, and as you said, check yes for outside lab and put the lab's name and address in box 32.
