When a physician is doing a review conference; meaning they are telling the patient the results of their skin test or any other test they may have done, what information needs to be included on the encounter in order for it to qualify for an E/M service code?

Well, they usually do the testing and then do the results the same day making them take up some time in the room. But, when they are charging an E/M service they aren't putting the HPI and the ROS in the encounter just the results and their interpretation. I had a discussion with one the doctors yesterday and she said that if she just steps in to tell the patient her results of the test she took then she should be able to charge whatever she wants without having to add in the HPI because it is just a Review Conference. The results aren't given over the phone only in person, because most of the time it requires Tx.
