Hello Everyone! 
I would appreciate confirmation on a couple rules regarding "incident-to" billing, as I have heard some variations over the years.
1) It is to my understanding that the physician who established the plan of care MUST be present in the same office (same floor)/suite in order to bill Incident-To under a PA or NP who is treating the patient for follow-up visits. Being "available by phone" or "in his/her office" (on a different floor) is NOT acceptable. Correct?
2) If the physician who established the plan of care works in a specialty group and (in his /her absence) one of the physicians from the same group/ specialty is available in the suite while the PA or NP is treating the patient for a follow-up visit, how would this be billed?
