Ok guys can you please help with this report??? I am thinking that I should be using 20552 and 77012, since there is no mention of the sciatic nerve.... what do ya'll think


CT-GUIDED PIRIFORMIS INJECTION

FINDINGS: THE PROCEDURE AND POTENTIAL COMPLICATIONS WERE EXPLAINED TO THE PATIENT AND VOLUNTARY INFORMED CONSENT WAS OBTAINED.

THE PATIENT WAS PLACED PRONE ON THE CT TABLE. THE PATIENT'S BACK WAS PREPPED AND DRAPED IN THE USUAL STERILE FASHION. SUBCUTANEOUS LIDOCAINE WAS INSTILLED FOR LOCAL ANESTHESIA.

UNDER CT GUIDANCE, A 20 GAUGE SPINAL NEEDLE WAS INSERTED AND ADVANCED TOWARD THE LEFT PIRIFORMIS MUSCLE. THE LEFT PIRIFORMIS MUSCLE WAS IDENTIFIED WITHOUT EVIDENCE OF BLOOD OR CSF OR PARESTHESIAS. SUBSEQUENTLY, A MIXTURE OF 4 CC OF .25% BUPIVACAINE AND 1 CC'S OF DEPO-MEDROL WAS INSTILLED THROUGH THE NEEDLE.

THE NEEDLE WAS REMOVED. THE PATIENT'S SKIN WAS CLEANSED AND THE BANDAGE PLACED. NO COMPLICATIONS WERE SEEN. THE PATIENT WAS SENT TO THE DTC WITH POSTPROCEDURE INSTRUCTIONS.

IMPRESSION: SUCCESSFUL CT-GUIDED BILATERAL PIRIFORMIS MUSCLE INJECTIONS.
