Place of service denial for 31240

My Doctor performs concha bullosa nasal endo frequently in the office. We have recently had this code denied by Aetna (Mail Handlers Medicare) for the place of service 11. When I call to question this no one can explain it to me and there is no "rule" about it. I know it is covered by Medicare in the office. Has anyone else had this issue and how did you handle it. I want to appeal it but have nothing to back up my appeal.
