Hi All -:confused:

It's been a CRAZY auditing week for me. Someone please explain to me....why it seems like every chart I have with dementia as the past hx is being code with 294.10????? Mind you in the record there is nothing pointing to 294.10, it just says, pt has history of dementia.

I wish I had a good answer for you, but I deal with similar issues. Physicians don't speak the same language that we do. To them, I think history is a relative term and most of the time I think they mean it is a currently active history. Yeah, I know, but keep the faith! Maybe someone else will have a good answer for us.
