Hi,

Any help would be greatly appreciated.
Our Physicians are employees of the hospital and bill under 1 tax ID number.
We are a cardiac center.
For patient's with multivessel disease, the thoracic surgeons want to do a Cabg LIMA/LAD and have the interventional cardiologist stent the remaining vessels either same setting or within the same admission. Since the CABG has a 90 day global, how could the interventional cardiologists get paid for their portion of the procedures?
I was reading up on modifier 58 and I'm not sure if that would apply because they are not of the same specialty. They are in the same group which modifier states they can be in the same group. The same specialty has me scratching my head.

Did you try a 59 modifier on the stent if on the same day as cabg plus the modifier for the vessel lc rc ld unless there stenting in the same branch i dont see why distinct procedural service could not be used. If its during the same session or admission if this is not the same vessel being bypassed it would not be considered global I dont think unless its a branch of the same vessel being bypassed.
