Q: Dr a in our practice did a bilateral entropion repair (67924-50) on 10/14/09, unfortunately, it wasn't done to perfection. So dr b in our practice took the patient back to the or on 10/22/09 and redo a bilateral entropion repair (67924) with left full thickness debridement (11041). It was coded, 67924-50-78 and 11041-78-lt. Dx: 374.00 and 870.1 but it got denied by medicare. Is there something wrong with the modifier? What could've i've done wrong?
 
Yes you can use a 76 on a surgical procedure. However I would have used the 78 befor the 50. The 78 to pull the procedure out of the global before the bilateral distinction.
