Can someone explain what cpt means by "Report only once per sequential infusion" for 96417

Example: If 3 chemo meds are given.
Med 1: 2 hours,
Med 2: 1 hour,
Med 3: 1 hour

96413 initial chemo, up to 1 hour for Med 1
96415 additional hour of same Med 1
96417 x 2 for the 2nd med 1 hour and 3rd med 45 minutes

How do I interpret the cpt rule here ? Is 96417 only 1 unit and never 2?
