our doctor is a PCP and sees patients in Nursing Homes also.
patient is staying in a nursing Home but the pt was in Hospice
ca i bill this to Medicare?
if yes how do I code this and what POS code should i use?

will it be right if i use nursing home codes(99304-99310) and the POS code 34?

I am not sure if this is right or wrong. Can anybody advise on this please...

You will probably need to contact your MAC directly for the correct answer to this. Different Medicare carriers may have different rules when billing these services. I would check with my MAC directly, just to be sure you are billing these correctly.
 
Hospice

Yes you can use the E/M 99304-99310 and 34 I work for a hospice company and those are the codes we use when our physicians go out to SNF & ALF
 
Yes, you bill Medicare.

As long as the PCP is not employed or paid by Hospice, you will file the appropriate CPT code along with modifier GV. If Medicare denies it, you can call them and ask them to re-open the claim with the modifier.
