How would you code liposuction to gain better access to av dialysis fistula??

md performs liposuction of arm (due to large arms) to gain access to a deep limb of the dialysis av fistula. The cannula was used all along the course of the fistula. The fat tissue was removed anterior, lateral, and medial to the fistula.

would cpt 15878 cover this? Not indicated for adiposity,lipidystrophy, etc......
md does not touch fistula so have ruled out cpt 36832......
or would it be best to go unlisted?


any ideas anyone?

This situation is different from the types of procedures I normally code. But, it is my undersanding that any time a procedure is performed for visualization purposes that it is considered incidental to the primary procedure being performed.

15878 says upper extremites and 36832 seems correct
