how would the following op report be coded?

Attention then directed to the medial aspect of the left calcaneus, where and incision was made and carried deep through subcutaneous tissue, with care taken to identify and retract all vital neurovascular structures. All bleeders ligated and cauterized as necessary. Obturator and cannula introduced from medial to lateral with a one lateral centimeter incision made, with care taken to identify and retract all vital and neurovasclar structures. The cannula left in place, the obturator was removed, and the endoscope introduced into the medial incision. The medial band of plantar fascia was clearly identified. Using a probe through the lateral incision, the medial band of plantar fascia was noted to be extremely tight. The probe was removed and the triangle blade used from the lateral incision to release the medial band of plantar fascia. The plantar fascia was released approximately 1/3 across the medial tubercle of the cancaneus. The toes were dorsiflexed and the muscle belly of the abductor hallucis was identified
