Physician codes are 36225-RT, 36223-59-50, 36226-LT.
Should I add 36228 to 36223?

Selective catheterization, right subclavian artery, cerebral angiography angiography with radiological supervision and interpretation
Findings: The right subclavian artery is patent. The right vertebral artery is patent in the cervical and intracranial segments. the basilar artery is occluded in the distal segment prior to the origins of the superior cerebral artery. The top of the basilar artery and both PCAs are are occluded.

Selective catheterization, right common carotid artery, cerebral angiography, with radiological supervision and interpretation.
Findings: The right common carotid artery fills normally. Both internal and external carotid arteries are patent. The right internal artery has mild stenosis at the origin. Intracranial segments of the right internal artery are patent. The posterior communicating artery is hypoplastic. The anterior and middle cerebral arteries are patent. Capillary and venous phases are unremarkable. No contrast filling of the top of the basilar artery is seen.

Selective catheterization, Left common carotid artery, cerebral angiography, with radiological supervision and interpretation.
Findings: The left common carotid artery fills normally. Both internal and external carotid arteries are patent. Intracranial segments of the right internal carotid artery are patent. The posterior communicating artery is a moderate size vessel. There is filling of the left PCA but no reflux into the basilar artery. The anterior and middle cerebral arteries are patent. The capillary and venous phases are unremarkable

Selective catheterization, left vertebral artery, cerebral angiography, with radiological supervision and interpretation.
Findings: The left vertebral artery is a large vessel. It is patent in the cervical and intracranial segments. There is slow flow due to occlusion of the distal segment of the basilar artery
