Hi.

Please help.

Left heart cath with LIMA, SVG to the OM and SVG to the RCA, PCI attempted however the guide cath would not seat well enough to perform the procedure. I came up with:

93459-26-59
93564
92920-RC-53

I would appreciate any feed back anyone has on this. Thank you.

the 93459 includes the 93564 if you have a cpt book for 2013 look on page 512 first paragraph left side of page and I would use the 52 modifier
