Physician documents DC dx: 1.Community Aquired Pneumoina 2. Strep Pneumosepticemia.

Patient does have positive blood cultures for streptococcus pneumoniae. No sputum cultures done.

I know this should be simple but how do I code this?

Current codes:
1. 481
2.99591
3.0382

Is this correct?? or should it be:

1. 486
2.99591
3. 0382
 
I would query the provider an ask if the is the patient has sepsis with streptococcus.

I'd use 038.2 and 995.91. Under code descriptors septicemia codes require a SIRS code. Being that the organism is ID'd you shouldn't need to use 486. 038.2 is more specific to the physicians dx (Strep Pneumosepticemia) than 481.

My opinion.
