The dx reads right hilar mass with widening of right secondary carina extrinsic compression of right upper lobe
 
For the right hilar mass, the ICD-10 code is R91.8 for lung mass NOS found on diagnostic imaging of the lung.

If the latter diagnoses of widening of right secondary carina and compression of right upper lobe are normal symptoms/findings for a hilar mass, then they are not separately reported. Just as you would not report a diagnosis of cough or excessive phlegm for a patient with a diagnosis of bronchitis. If the physician feels the latter diagnoses are above and beyond the normal signs and symptoms for a hilar mass, then you could report the lung compression with code J98.4, The R91.8 code covers the widening of right secondary carina already.
