Hi All,
The below mentioned OT notes was encountered by me in my hospital, as per my knowledge this not wedge excision, but doctor claiming this as- total removal of the right big toenail, and ablation of the edges of nailbed. What cpt code we can charge for this procedure.
OT NOTES:
The skin incision is made at the 2 angles of the toenail below the nail margins.
A rectangular flap of nail fold was reflected back, and the nail is avulsed.
The proximal part of the nail bed, which correspond to the extent of the lanula ,
is then excised down to the bone.
The flap is replaced and anchored by stitches at each side.
The dx is Ingrown Nail
