My doctor does nail debridements for Medicare patients. When do I use 11721 and when should I use the HCPCS G codes? I know about the Q7-Q9, I am just not sure where these G codes (G0245, G0246 and G0247) come in. Thanks!

The G0245, G0246 & G0247 codes are for use only with patients with documented Loss of Protective Sensation (LOPS). There is policy on the CMS website specific to LOPS than can help you. I would include the link, but their site is down right now.
