HPI:Eric July is a 34 y.o. male was seen with complaint painful calluses b/l feet.

Derm Callus
Ortho plantarflexed metatarsals
Objective:
Physical Exam
NVS intact b/l feet. No open lesions, no soi. There is painful tylomas sub met 1,5 b/l feet.

Assessment:

Tylomas. Foot pain bl. Plantarflexed metatatarsal Metatarsalgia
**
Plan:
**
Patient evaluated.
Antiseptic applied. Debridement tylomas b/l feet[/B]
