My provider listed these are diagnoses: 1. Gross Hematuria 2. Acute Cystitis with hematuria 3. Prostate Cancer.

This patient has been having recurrent infections. When patients have recurring infections are they considered new problems each time or are they considered established worsening? Patient is being prescribed a medication at this visit. So I was thinking to count the #1 and #2 together and only use the acute cystitis with hematuria as one established worsening for 2 pts, and then prostate cancer as one established problem stable as 1 point.
