Wiki POS question regarding multiple locations,,

Just a general inquiry regarding a practice with multiple locations.. If a provider see's a patient in one location & sends the same patient to another one of our locations ( for the same practice ) to have a test done because the original location doesn't have the machine to perfom the test,, would we need to submit one claim for the office visit EM & one claim for the test, to show that they were performed at seperate locations or would we submit the claim as one, merging the services? ( there is only one EM used, the other is done by a nurse & results sent to the provider ) thank you

Because box 32 of the CMS 1500 claim form needs to reflect the location where services were provided, if you are reporting services at multiple locations , you will need a different claim form for each location.
